Healthy Actions Medical Plan

Take care of yourself this year and get rewarded with the Healthy Actions Medical Plan. All you need to do is complete a few Healthy Actions by October 31, 2017, to participate. The same eligibility requirements apply to both Base Plan and Plus Plan members. 


Actions to take. To be eligible for the Healthy Actions Medical Plan (whether you are at the Base Plan or Plus Plan level), you must be benefits eligible, and complete the following actions between October 1, 2016, and October 31, 2017:

CLICK. Take the Cigna online Health Assessment at Log in (or register) "Take My Health Assessment" under the "My Health" tab.

CHECK. Get a checkup. An annual routine physical or County-coordinated wellness screening qualifies. You may either visit your own primary care physician or find information on upcoming annual physical dates (available in the Employee Wellness Clinic) and the 2017 Wellness Screening program.

CHOOSE. Complete THREE different actions from the following:
  • flu shot
  • 1 routine dental cleaning. If you are on the DHMO plan, please submit the HAMP Proof of Completion Form and supporting documentation for credit.
  • 1 routine vision exam
  • routine mammogram
  • 1 routine OB/GYN (Well Woman) exam
  • 1 prostate cancer screening
  • 1 routine colonoscopy
  • Harris County Employee 5K (must finish the 5K to qualify)
  • Live Healthy Harris County Challenge (must earn 200 points to qualify)
  • 2 hours of onsite County-coordinated wellness training classes
  • Naturally Slim program (must finish to qualify)
  • Cigna Healthy Pregnancies, Healthy Babies Maternity Program
  • Cigna Health Coaching Program (4 week min. to qualify)
  • Cigna Your Health First Chronic Disease Management Program
  • Cigna Apps & Activities (must earn 1,000 points to qualify)

Where to check your status. The Harris County Employee Information website is the place to check the status of the actions you complete. Go to and log in. Then scroll down to the bottom of the page and click the "Healthy Actions Medical Plan Activities" button to view your completed actions. Please allow a minimum of 60 days for your actions to be recorded.
